I have seen that failure with early Eriksson power LDMOS transistors and that
was because of a semiconductor process problem. I do not think that Motorola had
this problem so I suspect a transient over-voltage spike on the gate may have
caused your problem. If it were my amplifier, I would replace the transistor
before operating again because it will likely fail just when you need it during
a contact.
