[Moon] documents

Hello all I would like to  apply for high power permit but do not know how to word such a document if anyone has one in the UK could they help me to with a draft or some thing I would be greatful ,the bands I am looking for would be 144,432 & 1296 for eme experiments and the like I hope me asking is ok.
